Intake forms are completed online in the client portal.
After you schedule your intake assessment appointment, you will receive an email from us with a link to your personal client portal where you will find all of the necessary forms.
All forms must be completed in full and received at least three days prior to your appointment to ensure the best use of time during the intake session.
Once your portal account is established, you can use it to update or complete new forms and requests, among many other features. Access your portal account here.
